The patient portal and open notes

Patients book, fill intake forms, join video visits and read their own notes — the record itself, line for line, the same words the clinician wrote (Level 3 open notes), with visibility configurable per item where a result needs a conversation first; there is nothing to install and the account is the patient's verified phone.

What patients see

Not a summary of the note, and not a version with the clinical language taken out. When a clinician signs a note it leaves the desk as the record itself and the patient reads it that evening — same words, same order, same assessment. That is Level 3 open notes, the Mayo Clinic standard and the strictest of the three. Visibility is configurable per item where a result genuinely needs a conversation first: the default is open, and the exception is logged.

What patients do

Book appointments, fill intake forms before the visit, join telehealth visits, open requisitions the doctor sent, manage or reschedule a booking from the secure link, and leave a review after the visit.

Signing in

A patient signs in through the portal and is handed back on a single-use token. The first time they book they confirm their phone by text, and that verified phone becomes their account — it is how the health card, the intake form and the visit attach to the right person. No app; the portal runs in the browser at patient.qlynic.com.

For patients reading this

Your clinic is the custodian of your record. To access or correct your health records, contact your clinic; if you write to Qlynic directly, the request is referred to your clinic within five business days.
