Patient reviews
About an hour after the visit the patient is asked to rate it on five pomegranate seeds with an optional 500-character comment; the comment is read on submit by a keyword pass, so a four-out-of-five that mentions a safety concern is raised as urgent and on every open dashboard within the second, and escalating it is a person's decision that requires a written reason, emails the clinic owner, and is written to the audit log.

The ask
The request goes out about an hour after the appointment was due to end, while it is fresh. Five seeds instead of five stars, because a familiar control is an invisible one; a 500-character box that nobody is made to fill.
Read, not counted
The written comment is scored the moment it lands — a keyword pass, in well under a second, at no cost per review — and the review appears on every open dashboard in the clinic within the second. A score is not the decision: a four out of five with clinical safety language is raised as high regardless of the number, while a rating-only system would file it as a good visit.
Escalation
Someone at the clinic escalates it, and the software will not let them without a reason. The reason is stored on the review, the clinic owner is emailed with the reason and the name of whoever raised it, and it all goes to the audit log. An internal note (a thousand characters) stays internal; the patient never sees a word of it.
The average, honestly
A clinic can hide a review, and a hidden one drops out of the average and the counts — but every status change, that one included, is written to the audit log with who made it and what it was before. Reviews are received for the clinic's own review and follow-up; Qlynic does not publish, suppress or moderate public reviews.
