Describe what you want to ask in plain English and Qlynic drafts the questions — free text, multiple choice, a 1-to-10 scale, a date, a phone number, or a yes/no that opens a follow-up box when the answer is yes. One credit drafts one form. You review, reorder, reword or throw away anything before it goes live.
When it appears
Every form carries its own rule: a doctor (or the whole clinic), a trigger (every booking, first visit only, or when the booking reason matches keywords you set — checkup, pain, injury), and the booking collects only the forms that fit it. A knee injury and an annual physical can ask entirely different questions.
Filled before they arrive
The form appears during online booking, on the patient's phone, before the appointment record even exists; the submission is linked to the booking once it is made. By the time the visit starts the answers are on the chart, marked as coming from intake.
Required means required
A form marked required is put in front of the patient before the booking is confirmed, with nothing to dismiss it. If the save fails the form stays on screen and says so. Every required answer is checked again on the server, and a submission is written all at once — the answers and the record, or neither — so a half-saved consent form never exists.
Families
Each person on a group booking gets their own queue of forms, recorded under their own name and attached to the family's appointment.
