Lab, imaging and referral requisitions
A lab, imaging or referral requisition is written in the room — numbered, pre-filled from the record, printed on Alberta's own APL 21302 general laboratory requisition and sent to the patient as a secure link by text and email that is valid for thirty days; only a login linked to a provider profile can order, and only for its own appointments. Inbound results are not built yet.

Written before they stand up
Everything the record already knows — name, date of birth, health card number, phone, clinic, ordering provider — fills the sheet; the doctor writes the one line only a doctor can write. Each requisition carries its own reference number, a Routine / Urgent / Stat pill, and prints or downloads on the spot. Anything the record does not have prints as a blank writable line, so the sheet is never unusable.
The lab requisition is Alberta Precision Laboratories' General Laboratory Requisition 21302 (Rev 2024-09) itself — the top half pre-filled, the right boxes ticked, the collection band left empty because it belongs to the lab. There is no urgent box on 21302, so an urgent order is written at the front of Clinical Information in capitals.
How the patient gets it
A text and an email under the clinic's name, with a secure link to view, print and read the prep instructions, good for thirty days. If the visit raised a lab form, an imaging request and a referral, they sit behind the same link. Send it again and the previous link stops working. Patients who opted out of texts or emails are left alone, and when nothing can be sent the screen says so.
Who can order
Raising a requisition is not a permission you can hand out. It is a link to a provider profile — without one, nobody can order, not the practice manager, not the owner. A linked provider can only create orders for their own appointments, and every refusal is written to the activity log.
Open orders carry forward
An order nobody closed shows at the top of that patient's next chart, whatever the visit is for, and can be finished from there. It matches the patient on email address and shows the five most recent open orders.
