Yes — a family booked into one slot gets one encounter with a separate record per person, each keyed to their own patient ID; the doctor picks how a group's length is worked out (per person, fixed, or base plus extra), nothing comes out shorter than the doctor's own slot, and a group past the doctor's cap is refused rather than squeezed.

Four people is not four appointments, and it isn't one either. The doctor picks one of three ways to answer that: per person (30 × 4), fixed (60, whatever the size), or base plus extra (15 + 3 × 10). A group of one is just an appointment. Whatever the arithmetic produces, it is floored at the doctor's own slot length, and a group past the doctor's cap is refused outright rather than shortened.

Each person on a group booking walks their own queue of intake forms, recorded under their own name. On confirmation every member with an email address gets their own copy of the receipt, the calendar file and the reschedule link. A group moves together when rescheduled, and everybody is told.
