Signing in, passwords and lockouts
Clinic staff sign in on the Login page and can reset a forgotten password from there; five wrong passwords lock the account for five minutes and fifteen failures from one address in ten minutes block that address for fifteen, every failure gets the same "Invalid login attempt." message on purpose, a disabled member is refused on their next click, and patients sign in through the patient portal with a verified phone rather than on the clinic login page.

Clinic accounts
Sign in on the Login page with the clinic or staff email. A forgotten password is reset from the same place by email. Staff logins are created by the clinic — a member is invited by the clinic and accepts the invitation to set up their own login — and a login is not the same thing as a payroll record.
"Invalid login attempt."
Three different mistakes — an address never used here, a wrong password, or a patient account typed into the clinic form — deliberately get the same sentence, so a login form cannot be used to discover who works at a clinic. A locked account is told it is locked.
Lockouts
Five failed sign-ins against one account lock it for five minutes. Fifteen failed sign-ins from one public address inside ten minutes block that address for fifteen minutes, whoever it was aimed at. Wait it out; nothing else is needed.
Access removed
If the clinic owner disables a member, their next request is refused, logged and signed out. The identity cookie can take up to thirty minutes to fully expire, but every page has already refused it.
Patients
Patients do not use the clinic login. They sign in through the patient portal, where the account is the verified phone number confirmed by text at the first booking.
Errors
If something breaks rather than being refused, the screen shows one sentence and an eight-character reference like 4F2A9C71. Quote it to support@qlynic.com and the exact request can be found.
I cannot see or reset anyone's account; the Support page is the route if the steps above don't work.
